We will arrange a meeting with you and/or your NDIS Support Coordinator to discuss your needs and preferences. During this meeting, Share with us the supports you require, how we can assist you in achieving your goals and the type of support person who will best suit your needs.
We will develop a personalised care plan that reflects your specific needs and preferences. Your support should feel right for you, which is why we take the time to understand what matters most. We’ll match you with a support person who aligns with your goals, lifestyle and the qualities you value, ensuring you receive the best possible care in a way that feels comfortable and empowering.
We will provide you with a clear and detailed service agreement for your review and approval, ensuring transparency and peace of mind. This agreement outlines the support we will provide, tailored to your needs, so you know exactly what to expect.
Once your care plan is in place and your service agreement is approved, you can begin receiving the support tailored to your needs. Our team will be there every step of the way to ensure a smooth and comfortable transition into your new supports.
We will regularly check in with you to ensure your supports are meeting your expectations. Your feedback is important to us, and we are committed to making any necessary adjustments to ensure you receive the best possible care and support.
True care is committed to being an active member of your care team. We will attend and engage in your care team meetings with your support coordinator, allied health professionals and or family to ensure your needs are consistently met.
Participants, family members, Support Coordinators and Allied Health professionals can expect a collaborative approach as we work together to provide high-quality care and tailored support.
